Agency staff and credentialing committees cannot pre-approve any part of the
application including curriculum or specific documentation.
Social Work Licensure Requirements -
Read the instructions carefully and use the checklist at
the end of the application to be sure you have completed all required areas and
included all required documentation.

the form or application you need.
A completed general application
a completed supplement is required from every applicant. Please read the

Information regarding use of a clinical supervisor not employed by the agency
where you receive your work experience.
A fingerprint card or a fingerprint clearance card are required with every
application. If you need a fingerprint card and instructions please call
the Board at 602-542-1882 and leave your name and address to have them mailed to

issuance of a clearance card. You must contact DPS for information
regarding the process to obtain a clearance card.

The Social Work Credentialing Committee will review your application. If
your application is approved, you will receive testing information.
