Renewals


HB2361 will take effect on September 18, 2003.  This bill requires that all certified or licensed professionals, or applicants for certification or licensure, notify their regulatory board within 10 days of BEING CHARGED WITH any felony or misdemeanor that could affect patient safety. 

Licensure must be renewed every two years. Renewal requires thirty clock hours of continuing education and the submission of an Application for Renewal.  You are required to keep documentation of the thirty clock hours for 48 months.

Legal Residency Requirement
In accordance with federal and state law, each professional licensing agency in Arizona is now required to request proof of residency status from new applicants and current licensees at the time a license is renewed.  Instructions and the required form are now available on the Board's website and will be mailed with all future application and renewal packets.  Click here.

Renewal Application
This form can be printed and completed by hand or by directly typing in the form's fields.  At this time the form cannot be submitted electronically and will need to be printed, signed and mailed.

 

 


Credentialing Committee members and agency staff CANNOT approve any required documentation prior to receipt of an application.
