Information for Supervisees

For supervisees:

Each discipline has specific requirements for the qualifications required to provide clinical supervision and the amount of hours needed.  The discipline-specific requirements can be found below:

Social Work
Counseling
Marriage and Family Therapy
Substance Abuse Counseling

Your clinical supervisor must maintain their licensure throughout the period of clinical supervision and be compliant with the educational requirements.  You can verify licensure on the "Verifications" tab on the Board's website, and can view the Board's supervisor registry below which displays licensees who are compliant with the educational requirements.  Inclusion on the registry is voluntary, so if your clinical supervisor is not listed, it does not mean they're ineligible to supervise, but it's your responsibility to confirm they're in compliance with the educational requirements.

For clinical supervisors with a certification from a national accrediting body, it is the supervisor's responsibility to meet the ongoing requirements of the accrediting body to maintain the certification.  If the national certification is revoked prior to its expiration, clinical supervision hours provided to supervisees after the revocation will not be accepted for licensure purposes.

There may be situations where your clinical supervisor does not meet the discipline-specific requirements listed above, or the qualifications listed in A.A.C. R4-6-212, and you need to request an exemption.  An exemption may be requested pursuant to A.A.C. R4-6-212.01 at any time during the period of clinical supervision using the form below.

Clinical Supervisor Exemption Requests 
If you are requesting an exemption under A.A.C. R4-6-212.01(1)(c), you may be required to submit an additional exemption request form if it was required under previous Board rules. Please review the Applying for Licensure tab for your discipline to view the rules and applicable forms.

Supervisors and supervisees are responsible to ensure documentation of clinical supervision sessions meets the Board's requirements in A.A.C. R4-6-212.  A sample clinical supervision session form is provided below as a resource, but is not mandatory.  The form was developed through a task force between the Board and the Arizona Department of Health Services.
